You should be able to put everything under one server group since they represent the same DB. The audit/security policies would also be created considering the DB instead of individual nodes (unless you have a particular requirement to do so).

The three IP addresses listed above correspond to a single Oracle database server that is running as a "three-node RAC cluster." However, I would like more specific information about the following:

Alright, so after how many server groups, services, application do we need to define? Since each of the three on-prem instances mentioned above is for a single database server, how can I apply policy (security/audit) on it?
~> Is there anything else we can do to it?
~> What settings should we apply to make these three instances function as a single database?
~> If we define a scan on it in the future, which of the three instances listed above can we define the scan on?
